Job Title: Family Support Provider

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Mediate between the consumer / family and professionals
  • Provide Support / Advocacy for consumer and families
  • Complete SOC forms for every interaction with consumer / family
  • Be a member of the Family Team to empower families to meet their goals and objectives

Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education/Experience:

High school or equivalent.

Must be 21 years of age or older

Have experience as primary caregiver of a child or youth who has received services for serious emotional disturbance, or experience as the primary caregiver of a child or youth who has received services for substance use disorder and/ or co-occurring substance use and mental health, or experience of being a caregiver with Child Welfare/CPS involvement experience.

Reasoning Ability:

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Computer Skills:

To perform this job successfully, an individual should have knowledge of Database software.

Certificates and Licenses:

Must have the ability to attend Wraparound 101 online and 2-day in-person training, FSP Pre-requisite e-learning, 2-day FSP in-person training.

Supervisory Responsibilities:

This job has no supervisory responsibilities.
